Athletic Clearance Instructions
Complete the athletic clearance process using the instructions below.
Important: After your clearance information has been reviewed, you may receive an email with instructions for completing the required ImPACT test. Please check your email regularly, including your spam/junk folder.
Please make sure your physical is stamped and dated by the physician.
Must be signed by the athletic director before riding home with parents.
